Insurance Claim Form

For accident medical claims, it is important to communicate that the following is required before for any processing to take place:

  1. Completed claim form (signed by the named insured)
  2. Explanation of benefits from the primary insurance carrier
  3. Itemized bills from all providers (HCFA or 1500)

 

Camp Rates
Rates below are for all camps that will be hosted on ATU campus and per camper. 
Day Camps-$1.567
Weekly Camps (3 or more consecutive days)-$6.159
Overnight Camps-$8.565
Non-Sport Camps-$.374
