Required Forms
HEALTH RECORD:
The enclosed health record should be completed and returned. In case of illness the health form must accompany the camper to receive medical treatment. Please have the form completed before registration. A ball-point pen should be used in filling out the form to ensure legible copies. A Doctor's signature is not necessary, but parents must write in the name of the family physician.
