Test Registration Form

The Test Registration Form is needed prior to taking the Residual ACT.
You may fill it out on-line or fill it out at the testing center the day of testing.

Please Note:This form will not fully register you for the Residual ACT test. To complete registration you must pay $35 with cash, credit or debit at Student Accounts in Doc Bryan. You will receive a receipt for payment and the receipt will register you when you bring it to the testing center in Doc Bryan, Suite 171. You may also pay with debit or credit by calling Student Accounts @ 479-968-0271 or mail in a money order made out to ATU.
